The SMAS — the superficial musculoaponeurotic system — is the structural layer that surgeons address in a traditional facelift. It lies beneath the skin and subcutaneous fat, and it is the tightening of this layer that produces the most significant and lasting lifting results. Ultherapy reaches it without surgery.
Ultherapy uses US FDA-approved Micro-Focused Ultrasound with Visualisation (MFU-V) to deliver precise, high-intensity ultrasound energy to three distinct tissue depths: the deep dermis, the subcutaneous tissue, and the SMAS layer. The energy creates coagulation points at each depth, triggering an intense collagen remodelling response that develops progressively over three to six months. The treatment is performed with real-time imaging, so the specialist can visualise the tissue layers being targeted and confirm accurate energy delivery before each pulse.

Assessment and Mapping
Your specialist evaluates the degree of laxity, the tissue quality, and the depth of treatment required. The transducer selection and treatment map are planned before the session begins.
Ultrasound Gel
Ultrasound gel is applied to the treatment area to facilitate transmission of the energy and real-time imaging.
Real-Time Imaging
The specialist places the transducer on the skin and views the tissue layers on the imaging screen. Each depth is identified and confirmed before any energy is delivered.
Energy Delivery
The transducer moves across the mapped area in systematic lines, delivering focused ultrasound pulses at each target depth. The sensation at the SMAS depth is a brief, intense warmth. This is the deepest treatment in non-invasive aesthetics.
Soothe and Protect
The session concludes with a soothing serum and SPF. Mild redness and slight swelling may be present for a few hours. Occasionally, mild tenderness in the treated tissue persists for a few days.

How does Ultherapy differ from Thermage?
Thermage uses monopolar radiofrequency to heat the dermis and subcutaneous tissue, producing surface tightening and collagen remodelling Ultherapy uses focused ultrasound to reach the SMAS layer, producing a more structural lift. Ultherapy typically works at a deeper layer. The technologies complement each other and are sometimes used in combination.
How long do results last?
Results develop over three to six months and typically last twelve to eighteen months. Maintenance with other collagen boosting treatments and a healthy lifestyle and diet can increase the longevity of the results. Most clients return once a year to maintain the result.
Is the treatment painful?
Comfort measures like topical numbing cream applied for an hour prior to the procedure and /or oral OTC pain relief medication taken a couple of hours before the start of the treatment makes it manageable for most clients. Your specialist will discuss pain management options at consultation.
Who is a good candidate?
Clients with mild to moderate laxity of the face, brow, neck, or chest, who want a meaningful structural result without surgery. Your specialist will provide an honest assessment.
Is there downtime?
Mild redness and swelling resolve within a few hours. Occasional post-treatment tenderness in the SMAS layer may persist for a few days and is a normal response to the depth of energy delivered. Most clients resume regular activity the same day.
